Request for information can be made to the information officers mentioned in the Act No. 12 of 2016 using application RTI 1 (Note that completion of this Form is not a compulsory requirement to make a information and that any written letter, E-mail or verbal request with the essential  information to identify the requested information is sufficient) in order to obtain information in relation to the Sri Lanka Atomic Energy Board under the above Act Submission of the application is not compulsory
